Searches for "average surgery cost" and "how much is surgery" get vague answers because the honest answer is a range, and the range is wide. This page narrows it. Every figure below is a typical 2026 range from published sources: transparent-pricing surgery centers, ASPS surgeon-fee surveys, Peterson-KFF cost data, and patient-facing prices from accredited Colombian hospitals. None of it is a quote.

The one thing to understand first: a surgery is four bills, not one

When a US hospital tells you a procedure "costs" a number, ask which of these four it includes. Usually it is only one or two.

  1. Surgeon's professional fee. The operating surgeon's charge for the procedure and, usually, a defined window of follow-up (the "global period", typically 90 days for major surgery). See Surgeon Fees Explained.
  2. Facility fee. The hospital or ambulatory surgery center's charge for the operating room, nursing, supplies, recovery room, and any overnight stay. This is usually the largest line and the most variable.
  3. Anesthesia. Billed separately by the anesthesiologist or CRNA, calculated on base units for the procedure plus time. See Anesthesia Cost for Surgery.
  4. Everything else. Pre-op labs and imaging, pathology if tissue is sent out, implants and hardware, the assistant surgeon if one is used, and post-op visits outside the global period.

Why quotes go wrong. A surgeon's office quoting "$4,000" is usually quoting bill #1 only. A hospital estimate quoting "$18,000" is usually bill #2 only. Neither includes bill #3. Ask for all four in writing, and read how to read a surgery quote before you sign anything.

Why there is no single "average surgery cost"

Three separate price systems run in parallel in the US, and a patient can be quoted any of them for the same operation:

1. Chargemaster (list) price

Every hospital keeps a master list of charges. It is the starting point for billing and it is the number uninsured patients see on a first bill. Chargemaster prices are commonly 3 to 6 times what insurers actually pay. A laparoscopic gallbladder removal with a $6,836 transparent cash price can carry a $25,000 to $40,000 chargemaster figure at a large hospital. Nobody with negotiating leverage pays it.

2. Negotiated (insurer) price

What an insurer has contracted to pay. Since 2021, US hospitals are legally required to publish these, and since 2022 insurers must too, but the files are large and hard to read. Your share is your deductible plus coinsurance up to your out-of-pocket maximum, which in 2026 can exceed $9,000 for an individual plan.

3. Cash (self-pay) price

A discounted price for paying without insurance, usually 40 to 70 percent below chargemaster. Transparent surgery centers post it as one all-inclusive number. Hospitals usually make you ask. Then there is a fourth system outside the US entirely: the international private hospital cash price, which in Colombia, Mexico, Thailand, and Turkey is typically 50 to 70 percent below the US cash price for an equivalent procedure at an accredited facility. Why Colombia keeps appearing in these tables. The World Health Organization's 2000 World Health Report ranked Colombia's health system #22 in the world and #1 in the Western Hemisphere, ahead of the United States and Canada. That report has not been repeated, so for current verification pair it with two live anchors: JCI accreditation (a hospital-level credential; standalone clinics are not JCI-accredited) and ReTHUS, Colombia's national health-professional registry at rethus.gov.co, where any surgeon's license can be checked.

How to get the lowest legitimate price

  1. Get a written Good Faith Estimate. Under the No Surprises Act, uninsured and self-pay patients in the US are entitled to one before scheduled care. If the final bill exceeds it by $400 or more, you can dispute it.
  2. Ask for the self-pay price, then ask for the ASC price. Two different questions, often two different numbers.
  3. Check a transparent-pricing center. Surgery Center of Oklahoma and similar facilities post one flat number. Even if you don't travel there, it is your negotiating anchor.
  4. Get an itemized international quote. For elective procedures where you can travel, an accredited Colombian hospital quote is typically the lowest credible number you will see, and it arrives bundled (surgeon, facility, anesthesia, first follow-up in one figure).   5. Compare all-in, including travel. Add flights, 7 to 14 nights of accommodation, and a buffer. For procedures under about $3,000 US cash, travel usually eats the savings; above that, it rarely does.

Why is surgery so much cheaper in Colombia?

Lower surgeon and staff labor cost, far lower malpractice premiums, no insurance-billing overhead, cheaper facility construction, and lower implant distribution costs. Equipment, implants, and training standards at accredited hospitals are comparable. The gap is structural, not clinical.

Is a cheaper surgery a worse surgery?

Not inherently. A Surgery Center of Oklahoma knee replacement at $17,679 uses the same implants and board-certified surgeons as a hospital charging $35,000.
