Laparoscopic vs open: which costs more?

On the operating-room bill, laparoscopic usually costs more: specialized instruments, disposable trocars and staplers, insufflation, video equipment, and often longer operating time. On the total bill it usually costs less: hospital stays drop from several days to same-day or one night, wound complications fall, and patients return to work in one to two weeks instead of four to six. For an uninsured patient paying per inpatient night ($1,500 to $4,000 in the US), the laparoscopic version is generally the cheaper total.

Robotic-assisted laparoscopy (da Vinci and similar) adds $1,500 to $5,000 to a US bill. For most routine procedures the evidence does not show an outcome benefit over conventional laparoscopy that justifies the premium; it is worth asking whether the surgeon offers standard laparoscopy at a lower price.

Elective vs emergency

Appendectomy is almost always emergency surgery and is not something to travel for. Gallbladder removal is emergency in acute cholecystitis but elective (and therefore travel-appropriate) for symptomatic gallstones between attacks. Hernia, hysterectomy, reflux surgery, and bariatric surgery are elective. For elective laparoscopy at an accredited Colombian hospital, plan 5 to 10 days in country: surgery, 1 night, follow-up at day 5 to 7, cleared to fly.
